Family portals are switched on by the care agency, so the invitation always starts with the agency, not the software company.

Most UK home care agencies use software with three front doors: an office login for coordinators and managers, an app for carers, and a portal or app for families. This page collects the official sign-in links for each, so you can get to the right one first time.

A note before you start. HomecareOS is not affiliated with, endorsed by or connected to any of the other companies listed here. All product names and trademarks belong to their owners and are used only to identify their products. Every link was taken from the vendor's own website, help centre or official app store listing and checked on 23 October 2026. HomecareOS cannot give anyone access to another company's software. If you are locked out, contact your care agency or the vendor's support page linked in each section.

Birdie

  • Office login (Agency Hub): admin.birdie.care. Birdie's help centre says the Agency Hub uses a one-time "magic link" sent to your email instead of a password.
  • Carer app: Birdie, on the App Store and Google Play. Carers sign in with the mobile number their care manager has registered.
  • Birdie family portal: the Birdie Family app, which is the same Birdie app. The agency adds you to the person's care circle and sends a text invitation. You then download the app, enter that mobile number, type in the six-digit code sent by text and choose a four-digit passcode. Birdie's Family app guide explains the steps and says Birdie's support team cannot add anyone to a care circle.
  • Locked out? See Birdie's login help. If the app says your number is not registered, ask the agency to check your details.

OneTouch Health

Access Care Planning

Access Care Planning is part of The Access Group. It was formerly called Mobizio, which is why the web address uses mobizio.com.

  • Office login: your agency's cloud address, in the form cloudX.mobizio.com, where X is your agency's cloud number. Access's guide to linking Access Care Planning to Access Evo explains this. Staff whose agency uses Access Evo can also reach it from there.
  • Carer app: Access Care Planning on the App Store and Google Play. You need a server, username and PIN from your agency.
  • Family access: next of kin use the same Access Care Planning app. Access's next of kin login guide says you give the agency your device ID so it can register your device, and it gives you the server, username and PIN.
  • Locked out? See Unable to log in to the Access Care Planning app, or ask your agency.

Access CM (formerly CM2000)

Access CM is The Access Group's care management and electronic call monitoring system, used by councils and home care providers.

Webroster

Webroster is part of The Access Group.

PASS (everyLIFE Technologies)

CareLineLive

  • Office login: each agency has its own address, such as yourcompanyname.carelinelive.app. CareLineLive's contact page asks people to use the address their organisation gave them rather than searching for a login page.
  • Carer app: CareLineLive on the App Store and Google Play. Carers sign in with the server address, email and password from their agency.
  • Family portal: the Care Circle, which opens only in a web browser. You get an email invitation, which is valid for a week, and create a password. After that you sign in at the agency's own CareLineLive address, so bookmark it. See Accessing the Care Circle.
  • Locked out? See Trouble logging in?, or ask your agency to reset your password.

Log my Care

  • Office login (Care Office): careoffice.logmycare.co.uk, linked from the Log my Care website.
  • Carer app: Log my Care on the App Store and Google Play.
  • Family portal: the Family App, which uses the same Log my Care app. Log my Care's family portal terms say you must be invited by your relative's care provider. Log my Care says the Family App is available on its Pro and Outstanding plans, so not every agency will offer it.
  • Locked out? Contact your agency. Log my Care's help centre is mainly for care providers.

Nourish and CarePlanner

Nourish bought CarePlanner in October 2023, according to Nourish's announcement. Some agencies still use the CarePlanner rostering system alongside Nourish.

  • Office login: org.nourishcare.co.uk/login. Agencies on CarePlanner rostering use their own address, in the form yourinstancename.care-planner.co.uk.
  • Carer apps: Nourish Empower, formerly the CarePlanner app, on the App Store and Google Play. There is also Nourish Better Care at Home on the App Store and Google Play. Your agency will tell you which one to use.
  • Nourish family portal: a web portal, not an app. Nourish's FAQ for home care providers says the agency switches on access for a family contact. You then get a one-off passcode by text and an email with the link to the portal. See Better Care at Home for what families can see.
  • Locked out? Contact your agency.

Care Control

Unique IQ

Carebeans (QCS)

Carebeans is now part of QCS, and its apps carry the QCS name.

QuikPlan

Staffplan

Staffplan is OneAdvanced's home care rostering system.

  • Office login: your organisation gives you the sign-in details. We did not find a public login page on OneAdvanced's website.
  • Carer app: Staffplan iConnect on the App Store, and iConnect on Google Play.
  • Family access: we did not find a family portal on the vendor's public pages. Ask the agency.
  • Locked out? Contact your agency.

If you can't get in

Whatever the software, the same few steps solve most problems.

  1. Check you have the right door. Office logins, carer apps and family portals are separate. A carer login will often not work on the office website, and some family portals only open in a web browser.
  2. Use the link or address from your invitation. Several systems give each agency its own web address. Bookmark it the first time you sign in.
  3. Ask for a new invitation. Invitations and one-time links expire. The agency can send a new one.
  4. Contact the agency, not the software company. The agency decides who can see a person's records, and vendors' own help pages say they cannot add family members themselves.

Frequently asked questions

How do I log in to my relative's care app?

Ask the care agency which software it uses and whether it has invited you. Almost every home care family portal is opened by the agency, which adds you to the person's record and sends an invitation by email or text. Use the link or app named in that invitation, then sign in with the details you set up. The software company cannot add you itself.

Why can't I see the family portal?

The most common reasons are that the agency has not invited you yet, the invitation has expired, the portal is not switched on for that agency, or the person you support has asked for their information not to be shared. Some portals also stop showing records when a person's care ends or is paused. Contact the agency first.

Can the software company give me access to my relative's records?

No. The care agency controls who can see a person's records, because it is responsible for that information. Vendors' own help pages say access is set up by the provider, so contact the agency, which can check your details and send a new invitation.

Is the carer app the same as the family app?

Sometimes. Birdie and Log my Care use one app for carers and families, with different sign-ins. CareLineLive, PASS, Careberry and HomecareOS keep carers and families separate, and several family portals only work in a web browser. The table near the top of this page shows which is which.

I'm a carer and I'm locked out of the app. What should I do?

Use the forgotten password or new link option in the app first, if it has one. If that does not work, ask your office or care coordinator, who can reset your account or send a new invitation. Each section below also links the vendor's own help page.

Are these links official?

Yes. Every link on this page comes from the software company's own website, help centre or its official App Store or Google Play listing, and each one was checked on 23 October 2026. If a link stops working, the vendor may have moved it, so check the vendor's own website.
