Carer: report an incident or raise a concern

If something has happened, or something is worrying you, tell the office from the visit. In an emergency, call 999 first.

Live Every agency has this today.

Steps

  1. Open the visit and tap Report Incident

Choose Report an incident for something that happened, or Raise a concern for something you noticed.

  2. For an incident

Choose what kind it was, such as a fall, a medication error or a safeguarding concern. Say how serious it is, when it happened, and what happened in your words.

  3. For a concern

Say what you noticed and what is worrying you about it, and how soon the office should look at it.

  4. Say what you did

Optional, but it helps the office. Tick I have already phoned the office about this if you have.

  5. Send it

Tap Send report or Send to the office. It goes straight to the office.
